Health Care Powers of Attorney

A health care power of attorney is a written document naming another person as a representative to make medical decisions if an individual is unable to make them. Some states refer to these documents are health care proxies or health care surrogate designations. Instructions could include desires for types of treatment under various circumstances, similar to a living will. Health care powers of attorney typically permit agents to make most, if not all, decisions concerning health care when the principal is unable to make medical decisions or provide informed consent.
